Hours: Monday-Friday 8:00-12:00 and 1:00-5:00.
Certified copies of Coos County birth and death certificates can be obtained from
Coos County Public Health, Office of Vital Records, for *six months following the
event. They may be obtained either in person in our office, or requested through
the mail. In person requests require an application form completed and photo ID presented.

To request a Birth Certificate from us by mail, please include the following information in your request:

- Birth name, date and place of birth
- Father's name and mother's maiden name
- Your relationship to the person of birth
- What the record is needed for
- First Copy $20.00
Additional Copies (ordered at same time): $15.00

payable to Coos County Public Health

To request a Death Certificate from us by mail, please include the following information in your request:

- Your relationship to the decedent
- What the record is needed for
- $20 (per copy) check or money order, payable to Coos County Public Health
*After six months, the records are transferred to the State and
need to be obtained through them at the following address:
