CaCoon
(Care Coordination)

CaCoon is a program that provides services for children from birth to 21 years of age who have special health or developmental needs. The program helps these families become as independent as possible in caring for the child. It also helps families access appropriate resources and services.

Public Health Nurse home visiting for children with special needs provides health education, family and child assessment, advocacy and parental support and training.
