 If you have vomiting, upset stomach or diarrhea and think you got sick from eating food that came from a food service inspected by CCPH, call 541-751-2431 or email to EH@CO.COOS.OR.US and report the following:

  • Your name and contact information
  • Name of person sick and their contact information (if you are reporting for a friend or family member)
  • The symptoms you are experiencing (what they’re like, when they started, how long they lasted)
  • What you have eaten that may have caused the illness
  • Where did you get the food that you believe made you sick (restaurant name & location)
  • Why do you feel your illness is caused by food poisoning

Reporting illnesses will help us find and correct problems in food preparation and this can protect other people from getting sick.  

As allowed by law, your personal information is confidential. False reporting is subject to legal ramifications.

What are the symptoms of food poisoning?
Symptoms may include:          

  • Diarrhea (may be bloody)
  • Fever and chills
  • Headache
  • Nausea and vomiting
  • Weakness and fatigue

When to Call a Doctor
Make an appointment with your doctor if you have:

  • Blood or pus in your stools
  • Stomach pain that does not go away after a bowel movement
  • Symptoms of dehydration (thirst, dizziness, light-headedness)

  • Diarrhea with a fever above 101° F (100.4° F for children)
  • Recently traveled to a foreign country and developed diarrhea

Seek emergency care if:

  • Bleeding is excessive or your stools are maroon or black
  • You may have poisoning from mushrooms, fish, or botulism
  • Your heart is racing, pounding, or skipping
